Since 1978, families in northern Colorado have turned to Hospice and Palliative Care of Northern Colorado at a time when having the support and expertise of hospice staff is profoundly important. We provide care in patient’s homes, in long-term care facilities and at our In-Patient Care Center, located on the fourth floor of North Colorado Medical Center. As a comprehensive program, hospice care considers the unique physical, emotional and spiritual needs of each individual. Working closely with patients, families, medical professionals and caregivers, our expert and caring staff focuses on helping people who are terminally ill live in comfort and with dignity. Hospice and Palliative Care of Northern Colorado, Inc. is a non-profit, community based hospice, serving rural and urban communities of Colorado’s Weld and Larimer Counties. Each community’s needs are met without regard to race, color, religion, age, gender, sexual orientation, handicap (mental or physical), national origin, diagnosis or ability to pay.
HPCNC has played an important role in our communities for 30 years. Year after year our customer service remains the hallmark of our organization. When people hear hospice, our organization comes to mind. We provide our patients with high quality end of life care, aimed at reaching each patient’s goals. We educate both patients and families regarding end of life issues, helping them to make better decisions and make the most of the lives they have left.
